Application Checklist

 

The following is a list of items which must be completed and submitted by all candidates seeking the status of Diplomate, American Board of Craniofacial Pain. Please click "Begin Application" above for required downloadable forms.

☐    Signed and notarized Diplomate Affidavit.

☐    A copy of your current dental license with expiration date. (Note: If your dental license will expire before the examination date, a renewed copy of your dental license must be submitted to the ABCP by the application deadline.)

☐    Patient List

A list of one hundred (100) patients whose chief complaints included Craniofacial Pain of non-dental or alveolar origin for whom you have completed all aspects of diagnosis and treatment. To insure privacy, the patient list documenting completed cases may include patients' initials or another approved coded format.

☐    Continuing Education Credits

A list totaling at least 500 hours of Craniofacial Pain "related" continuing education credits which you have completed within the immediate ten (10) years prior to the date of submission of your written application.

☐    Letters of recommendation/sponsorship obtained from two (2) current Diplomates. (Note: If you are reapplying, you may reuse the letters of recommendation you submitted with your original application.)

☐    Three (3) Patient Case Histories (to be presented during your oral examination). (Note: If you are reapplying, you may reuse only one (1) of the case histories you submitted with your original application.)

☐    Payment of the Application Fee

  • Applicants must make payment of $1,450 ($500 non-refundable application fee + $950 examination fee) upon application submission. This can be done securely online, by phone at 630-735-1405 Mon-Fri 8am-5pm Central Time, or by check made out to ABCP and addressed to 2574 Oak Trails Dr. Aurora, IL 60506

☐    If Applicable: A written explanation of a personal disability for which we must make special arrangements to accommodate you at the examination plus a letter from a qualified professional documenting your disability.

related courses

 "Related courses" shall include courses that concern pain management, neurology, physical medicine, pain and nutrition, osteopathic or chiropractic manipulation, radiology, orthodontics and/or functional orthopedics, psychology and pain, electromyography, biofeedback, acupuncture, occlusion, basic sciences as related to pain management and the neuromusculoskeletal system, practice management and pain practice, prosthetics and TMJ dysfunction, diagnosis and treatment of TMJ dysfunction, assessment and management of sleep-disordered breathing, cranial osteopathy, electromodalities (as applied to the head, face and neck), cervical orthopedics and/or cervical physical therapy; head and neck anatomy (post-dental school); and cephalometrics.

Credit hours for lecture and participation courses will be approved on a similar basis as that of other Specialty boards.

Courses which do not fall into any category specified herein, but which are nevertheless "related" to the practice of Craniofacial Pain, may be approved by special permission of the Board of Directors for purposes of fulfilling the continuing education requirements.

All decisions of the ABCP Board of Directors regarding the approval of related continuing education courses that may be used to fulfill prerequisites for Diplomate status shall be final.
